Oklahoma

Oklahoma gives members a direct way to request the benchmark.

Oklahoma's law covers qualifying direct payments accepted as payment in full, with no later balance bill. Behavioral health and durable medical equipment are included; prescription drugs are excluded.

State-specific mechanics

Do not reuse another state's checklist.

  1. Request the plan's average allowed amount in writing before paying.
  2. Get written confirmation that payment is accepted in full.
  3. Confirm a claim for the service will not be submitted to your health plan.
  4. Keep electronic documentation and the provider's network status.
Check the benchmark

Use the source this state or plan provides.

Oklahoma requires the plan to provide its plan-specific average allowed amount to an enrollee on request. Ask for that exact amount in writing before paying. Cash prices, Medicare references, and generic negotiated-rate files are not substitutes.

Before you pay

Get the price and service codes in writing.

If you are not using insurance, ask for a written Good Faith Estimate. Federal rules generally require one when you request it or schedule care at least three business days in advance. This price document is separate from the state credit process. Read the CMS guidance →

How submissions work

Start with the published carrier instructions.

BCBSOK publishes a purpose-built instruction form. Other carriers may use different document channels, so confirm the process with your own plan.
